You can request your official Sacramental Certificate(s) by filling out this pdf version of the Sacramental Request Form and submitting it by email to Katye Miller or by mailing, faxing, or dropping it by the church office. If you have any questions, please feel free to contact Katye.

Please note that in order to protect the confidentiality of these records, certificates will only be issued to the name on the certificate, or to a parent or legal guardian of the person named on the certificate, or to the requesting parish or diocese. Your signature is needed on the request.
No certificates are issued for genealogical purposes.
